Download the form to your desktop and save often. Send your completed form to the appropriate district office (https://nrb.vermont.gov/act250-program/district-staff-and-commissions).

This form provides applicants, parties, and others the opportunity to give us important feedback on the Act 250 permit process. If you have participated in the permit process and you have comments, suggestions, or other feedback, please download the form, complete it, and return to our Central Office in Montpelier.
